Survey regarding the perceptions of users, producers and others who work with 3-D printed prostheses

We are students from Midwestern University’s occupational therapy program. We are conducting a research study on 3-dimensionally printed prosthetics used by children with congenital upper limb deficiencies. We would appreciate your participation in our survey regarding the perceptions of users, producers and others who work with 3-D printed prostheses. We want to learn about your experiences and views regarding the current and future use of these devices with children who have congenital upper limb deficiencies.

Your participation in this study is completely voluntary and you can withdraw at any time, for any reason, without any consequences. If you choose to participate, you will be asked to complete an online survey that should take no longer than 10-15 minutes. Participants must be age 8 or above to complete the survey. Additionally, if participants are between the ages of 8 to 13, they should ask their parents for permission and for clarification as needed.

The physical risks of participating in this study are minimal and are about the same as the risks of everyday life. You may experience some emotional discomfort while reflecting upon your experience of limb loss. You are free to choose which (if any) questions to answer. Additionally, you may opt out at any time.

At the conclusion of the survey you will be invited to participate in an optional interview. A limited number of those who express a willingness to participate in interviews will be contacted or may contact one of the researchers to complete face-to-face or telephone interviews. The researchers will contact the participants a second time to validate that the findings of the study are consistent with the thoughts and feelings of the participants. To help protect your confidentiality, the survey will not contain any information that can personally identify you. In addition, any data collected will be stored on a password protected electronic device/computer. The finalized results of the study will then be disseminated via a poster board presentation at Midwestern University and possibly through publication in a professional journal.

You may not have a direct benefit from participation in this study, but the data from this survey will contribute to the conclusions of this study which will be distributed later.
